  2. SEND a self stamped-addressed DL size envelope to MEAG and we'll post a Registration Form to you.
  3. START photocopying your medical records and documents.  This is a big stumbling block to overcome but it is necessary if you want help.
  4. FILL in the Registration Form, sign the Consent, gather up your documents, then...
  5. RETURN the ORIGINAL form to MEAG by post, together with your payment and photocopied documents.  We do not accept registrations by email.
